When to file an incident report
Every time a player is removed from play or hurt, at a game or a practice. That includes every suspected concussion: any athlete showing signs or symptoms after a hit or fall comes out, does not return that day, and needs written clearance from a qualified provider before any return to activity Sourced.
- Keep blank copies in the medical binder at the check-in tent, next to the first aid kit and the venue emergency action plan.
- Fill it in on site, while the official, coach and site supervisor are still there.
- Notify the parent the same day and write down when, and by whom.
- Send a copy to the league safety officer within 24 hours.
- Call the family 48 hours later and record the outcome. If a concussion was suspected, attach the return-to-play log and the clearance letter when they arrive.
- Keep the records until the player reaches majority plus your state's statute of limitations.
The full removal and return process is in the concussion protocol, and the venue plan behind the response is in emergency action plans.
